1. The Duplicate Copy only be issued in the name which appeared on the original certificate.
2. This application is for the certificate only, excluding transcripts. To apply for transcript you may refer to this link: TRANSCRIPT APPLICATION | Academic Management Division (AMD)
3. Please note that any Duplicate Copy issued it is a replacement it may be not in the same format as your original certificate which has been lost or damaged.
4. University reserves the right to not approve the application a duplicate certificate is incomplete or for any reason deemed necessary.
5. Approval will be subject to Academic Management Division (AMD).
6. The applicant may contact this office after 21 working days of submitting the complete application to determine the status of the application.

Application for duplicate certificate shall be in accordance with the conditions prescribed by the University as follows:

1. Complete the application form.
2. Have a copy of the police report clearly states the loss / destruction of the degree / diploma and been verified.
3. Makes oath taking in front of Commissioner of Oaths (enclosed with this application).
4. A verified copy of identity card / passport.
5. Application fee is RM 500.00 (OF MALAYSIA: FIVE HUNDRED ONLY). Payment can be made online at PAYHUB UTM portal. Personal cheques are not accepted. Personal cheques are not accepted.
6. (Optional) Payment for postage charges can be made via PAYHUB UTM portal. Payment rate as below:

Update on application will be notify through email. Do Not send multiple e-mail. You are suggested to reply the same e-mail to avoid high volume e-mail inbox and will delay the process.
